I have actually done this before. Just go down to your local court house and tell the clerk what it is you would like to do and they will give you the necessary paperwork. There will be a fee, usually not very much, and you will be given a court date. As long as the mother and father are present and agree on the terms, the judge will sign the order and you are good to go!

How do we change a childs last name
My son and his girlfriend had a child together. Peternity has been established and the baby is now 16 months. The mother named the child with her last name but now has agreed to hyphon with my sons last name. He does not know how to start the process. can you help. We live in Indiana
