Grandparents filing for full custoday

My grandson is mentally disabled. My daughter has had sole custody since birth. Dad puts on a show, but never showed for any hearings. My daughter is exhausted, and cannot take care of him with the needs he has, she has been raising him alone, with alot of help from us, and has been considering placement in an institution to ease her stress. My husband and I don't want to have this happen, as we want to exhaust any options before taking any step like that, and want to be granted full custody of our grandson, and our jobs are willing to work around his special needs so he is able to get the attention necessary that he requires. Our daughter feels as well that this is the step to take and has agreed to granting us custody. What forms do we start with? We live in Madison Co. NYS, our grandson currently reside with his mom, but we want to take chare of his care beginning the first week of June. He is only five years old.

I would like to know where you found the papers online. I am going through the same thing as you and we are having a hard time finding the legal documents without paying an arm and a leg. We have spoke to both parents and they have agreed to the sign over, so I just need the proper forms. I f you could let me know it would be a very big help. Thank-you.
