Gifts in kind form

I am working on creating a "gifts in kind form" to give to those who donate to our organization things other than cash. Is there a basic amount of information I need to have on the form?

I would add what the item(s) were and an estimated value. Ensure to put on the form, estimated value. Name of your group with address info, and a place for them to sign as the donator and a place for you or your representative to sign. Make it a carbonless form 2 copies, you get orginal they get copy. I would neven give the orginal to the donator because they could change information. I would also put down the IRS reference for donated items for tax purposes at www.irs.gov and search in kind donations. There are numerous articles
