Need affadavit form for rebate of extended auto warranty

I purchased auto from dealer on 9/29 with extended two year warranty. Need an affadavit form (notarized) acceptable in Ohio to cancel this extended warranty. Other than stating mileage of car at time of cancellation, what other info do I include in this affadavit?
