Did you get injured at work? If so contact workmans comp. If not, get your doctor to write a note saying what you can and can't do. If you are a good employee they should work with you. Also ask to be put on light duty - your pay may drop, but you will have less hours and less physical work. Keep in mind though that they do not have to keep you as an employee if you cannot do your job requirements anymore. If you were hurt at work then they do, but if you got hurt on your own time they are not obligated to continue your employment.
