How do I change my daughters last name from her dad's to mine?

My boyfriend wants nothing to do with my daughter. I want to change her last name. He doesn't care. He is trying to sign his rights away to her. My daughter is 1 year old now.

Simply go down to the courthouse and tell them you want to file a name change. Then pay the fee and when everything is changed order a new birth certificate and social security card for records.
