How to change a infants last name to her fathers.

I want to know how to change my childs last name that is oneyear ols to her fathers. Her fathers wasnt around when she was born i didnt know that i could put his name on there. Now we want to get her name changed to his last name and i am all for it.

It's a simple form you can get from the county clerk. Say you want to amend the birth certificate. They can tell you how to change her name as well.

Go to your court house and ask for a last name change it should be around 56 dollars for the license
