I had my tubes tied at age 21, how much does it cost to have them untied?

Which types of insurance covers that procedure? I am looking to have my tubes untied and anxious to get the ball rolling.

I know that most major insurances will cover it:

Kaiser
Blue Cross
United Health Care

You can just call your insurance customer service and they will know off the bat whether it is covered or not. Trust me - I had a baby last year and spent a lot of time on the phone with them :)

So, for those who have had their tubes tied and it was paid for by Medicaid, will Medicaid also pay for the tubes to be untied?
