You don't necessarily need a form. If you wish to give the mortgage company authorization to release information to someone, simply write a quick letter that references your name, loan number, property address and lender name. Then state that you hereby authorize the mortgage company (name the company) to release your loan information and/or discuss your loan information with the person (name the person). Make sure the letter is dated and signed.

Most mortgage companies are fine with this standard letter as it covers everything that needs to be said. In the event that any company needs a certain 'form', they will let you know.
