Go to your state Secretary of State website. Click on infomation regarding LLC's, setting one up. There is usually a plethora of information there. They usually have forms you can get right off the site, including the Articles of Organization you need to fill out and send in. Educate yourself on the difference between "incorporating" and setting up an LLC. I have found the Secretary of State websites invaluable in my paralegal work in assisting with setting up and dissolving corporations and LLC's. If possible have a lawyer side check your paperwork. You will need to get an EIN from the IRS, which you can do online or by phone, using an SS-4.
Find legal forms has llc forms for each state.
