Grounds for changing Custody in IL

When my ex and I were going though our divorce, we'd agreed that she would be the primary caregiver for our 5 year-old daughter with a Joint Custody agreement. Some other things that we'd agreed upon in e-mails include not allowing our daughter to meet boyfriends/girlfriends unless it's obvious that the relationship is going to lead to marriage. Mostly because neither one of us wanted her to be meeting and getting attached to people that weren't likely to be a permanent fixture. I followed these rules faithfully because I don't know who my ex was going to be bringing home and exposing my daughter to. I'm very protective of my daughter and don't want her to be put through heartache and disappointment (or worse). Now that I am engaged and my daughter is spending time with my fiancee, my ex has completely reneged on the arrangement that we'd made. There have been several occasions (that I've witnessed myself), where my daughter has been exposed to my ex's current boyfriend. I've confronted her repeatedly about her hypocracy regarding the matter, and she simply says "I'm not going to follow through with the agreement. Get over it". She has repeatedly shown bad judgement in regards to my daughter's well-being. She pawns my daughter off on her grandparents so that she can go out on the weekends AND during the week, when she could easily hold off on her dates until weekends that I have my daughter. My ex also has a history of depression, for which she is taking medication, and (I believe) undiagnosed bipolar disorder.

These things are just a tip of the iceberg.

I honestly don't know what to do. I feel like she's ruining my daughter. I love her very much, and would do anything for her. I want to get custody of her so that I know for sure that she's being raised right. What are my options?
