How do I legally give physical custody of my son to my ex-husband?

I have been having many problems with my son. After discussing them with his father, he and I have agreed that he should go live with his father. His father lives in another state and I just want to make sure that I cover myself as far as the custody goes. I want to have something in writing that gives him physical custody.

He will go to the county court and file for full or sole custody (sole custody would mean he can make decisions concerning the son without your consent, but you can still have visitation). The county clerk can give him the correct forms and take the filing fee and when he goes to court the judge will turn over custody to him.
