Parental transfer of custody child

(mother of child) lives in kentucky, father in afganistan, mother has allowed child to be taken to georgia by childs grandfather; nothing legally binding- just a temporary visit. mother, experiencing a couple legal and health issues, is fearing the grandfather will attempt to assume permanent custody but wishes this not to happen. her wish is to have child relocatd and placed in the custody of her mother, the paternal grandmother.
question: from the vantage point of the mother and grandmother, what can and should be done to see that the situation changes legally to the satisfaction of the mother and grandmother. are there any immediate steps to be taken and how do they bring this together.
