Who is the petioner on the name change for minor form

If I the grandmother and my husband the grandfather have guardianship of our grandson and the grandson wants his last name changed to ours and the natural mother gives consent to this. who is the petioner on the legal name change form for minor?

We live in the state of nevada. we are the grandparents and have guardianship of our grandson.
the natural mother gives her consent to change his last name to ours. on the legal forms,are we the grandparents, the petioner?

You would be because you have legal guardianship so you are allowed to make legal decisions on his behalf.

On the first page of the legal form, there is a paragraph that says he or she is the natural father or mother. How would I fill out that paragraph. On page 1 it says comes now petioner_______
for his or her petion to changethe name of his or her natural mother or fatherof _____the child. Do I put her, the natural mother on that line? Or do I put myself as the petioner and write in Grandmother?

The only way you can give the grandchild your last name would be if the parents were to allow you to adopt. Guardianship has nothing to do with name changes. The father would also have to notified of this and agree.

Grandparents have know legal rights,however if you fear the the child is in danger and can prove it you could file a motion with the court and ask for tempory guardianship. It is hard to do, you would just about have to know when shes doing anything wrong and be able to report it when if happens. If CPS is ever involved they will always call the nearest relative to take the child,like a granparent. In the mean time I would try not to get my daughter upset if she does have a medical promblem. Hopefully she will turn to you when she knows it is to much for her to handle. Be there for her so that she will know that she can turn to you. This is not easy but it is for the best of the child. Good luck sounds like your granson has two wonderfull grandparents.
