Just want to confirm if her primary disorder is anemia & bulimic & if psychiatric consultation is being recommended for this client

Client is 14 yrs old female complained of fatigue & loss of stamina. Appetite was marginal & very conscious of maintaining body weight at 96 pounds. Menstrual flow was always heavy & long form onset at twelve yrs of age. Laboratory findings Hematocrit 28%, Hemoglobin 9g/dl, iron 16ug/dl, bone marrow iron absent, erythrocytes small & pale.

I am guessing you are working on this for a college course? is thier evidence of purging? Such as rotting of the back of her teeth, from vomiting? Some of the things they look at as far as tests for bulimia are complete blood count (CBC), as well as more-specialized blood tests to check electrolytes and protein, and liver, kidney and thyroid function. A ua may also be done. Approximately 50% of patients with bulimia have irregular cycles or amenorrhea.

Both her hcg and hct are below normal, so she is probably anemic. Secondary anorexia is probably a more accurate diagnosis. Why? because you were looking at bone marrow iron. How is that obtained and why? It is a somewhat painful procedure where they aspirate marrow from eiher the sternum, femur or iliac crest of the hip. The clue here is the distorted erythrocytes (rbc's) Therein lies your diagnosis. Good luck! Susu
