Lower left stomach pains

So for the last couple years i have had sharp intense pain in my lower left ab. It only happends once like every other couple weeks, sometimes months. The pain intensifies when i have to urinate. This is such intense pain to where i am hunched over almost in tears. (I am a 6 foot 6 290lb former defensive lineman) It almost feels like i pulled something there.
