I was told i need cataract surgery and the doctor will put in lenses my insurance will pay the surgery for cataracts but not the lenses do i have to

May I print those forms and submit them or do I just complete them on-line and submit them?

I have a rifle scope that I want to desolve the bond between the lenses and then reglue them together.
