Power of Attorney

I want to become power of attorney for my sister. She is mentally disabled and currently lives in another state. She will be moving to my state to live with me. I want to be able to change her address with Social Security and Medicare/Medicaid, but they won't do it without speaking with her. What kind of forms do I need and do I need a lawyer?
