You have to prepare and file a quit-claim deed. This is best done by a paralegal or an attorney. If you check some of the larger realty business in your location you will find one that can get it done for you probably for less than $100.
Be advised however, that if both you and your ex signed the mortgage papers for the home loan, a transfer of title to your ex will not take you off the hook for being equally responsible for the mortgage balance. In order to alleviate that responsibility, your husband would have to refinance the home in his name alone.
