Should you include another nurses name in your charting?

Should you add the name of the nurse who gave you report in a patients chart?

I am a nurse and I was recently inserviced,if you will,on this very subject.When writing in patients' charts never write the nurse proper name(name given at birth) or a certified nursing assistants name in the actual body of the nursing note.Instead you may write example: This nurse or This writer or This CNA,as part of your narrative.As we know all patients charts are considered legal documents which are subject to be used in court if needed in legal issues regarding the patient in question and one has to be very careful in what words you choose to use while documenting.Hope this helps to answer your question.
