Are you planning to allow another person to adopt your child? You need to contact the child welfare department, in OK, and ask them what steps you need to take, in order for the adoption to be legal. I believe that all states require the adopting parent(s) to complete home studies, adoption interviews, background checks, etc., before they can adopt a child.

You can get a lot of the forms online at a website called Oklahoma County (dot org) Court Clerk (slash) Forms.
These include the county court clerk forms, such as Certificate of Adoption, and the Affidavit of Nondisclosure (parents giving up rights).
Of course you should seek the advice of the court, or an attorney or adoption agency, but you know that. So I wish you the best of luck in your adoption procedure.
