Can I file a small claims complaint against this insurance company?

I live in Glendale, AZ. We took out a family policy with Cinergy Health. On the phone they told me my drugs would cost X. They quoted and confirmed the prices with their pharmacy people. When I went to fill Rx, the cost for the quoted drugs was several hundred dollars more than quote. We requested cancellation of policy and the four months of premium we paid (approx $2000) on or about May 7, 2008. Cinergy recorded the original phone call where they misquoted drug prices and we agreed to purchase policy. They said they would listen to the tape and get back to me. They did that today (7/15/08). Cinergy admitted on phone today they misquoted drug prices but will only return 1 month premium. They claim Rx is only a portion of coverage so they won't return all premiums paid. Cinergy played the recorded call for me on phone today. I clearly stated in the phone call "the drug quote will either make me take or not take the policy". Since they did not do what was promised to me on the phone regarding my drug coverage, I believe our entire premium should be returned. I want to sue in small claims court. They are a Florida company. Can I do this? If so, is it done in Justice Court? Superior Court? Are these forms online? Thank you for your assistance. It is much appreciated.
