How to add a family member to a house title

The title is under my name (in Texas). Currently there is a mortgage on it. I would like to add two family members to the title with rights of survivorship. What form(s) do I need to fill out and what do I do with the form(s)?

Also I have heard that with this arrangement any credit card debt from a member in the title "may" impose liability on the house equity after death (if any): is this true and if so is there a way around it?
