Release to copy a copyrighted DVD?

The original people who made me a DVD of a show I performed in had "copyright" printed on the label of the DVD eventhough they never registered the production w/the copyright office or obtained a Master synchronization license for the production. My DVD does not play in any machine or computer I own and was told by a production company near where I live they could more than likely find a machine or computer it will play on and then burn me a copy that will play in my DVD player or computer. But, I need to get permission from the original people who produced the show and DVD. I guess what I need is some type of copyright release form. I have been all over the internet, but have found nothing close to what I think I may need. Any suggestions on where to find something like this or what wording would be needed in a release-type letter?
