How do i program a .012 rad on a 1.268 dia. w/ a .03 rad insert

Whats the formula? for a lathe and mill.

If your tring to make an inside rad. it can't be done unless the insert has a .012 rad. or smaller, an outside rad. can be done but I would need more information as to the dims. of the part your tring to make, like where your X,Y,Z zeros are set and the distance to the centerline of the .012 rad. from said zeros.
