The reason puppies get a series of vaccines, typically starting at 6-8 weeks and continuing every 3-4 weeks until about 4 mos of age, is to overcome maternal antibodies in the system. At 4 mos, your pup doesnt have any maternal antibodies, so will only need a series of 2 vaccines for the classic DHLPP. I would have the dog examined (more important actually than the vaccines) ASAP and get the first DHPP and Rabies. 3-4 weeks later a follow up DHPP will be administered. Optional vaccines (should be given based on where you live and what the dog's lifestyle will be) include Lyme, Leptospirosis (a common one to cause a reaction) and Bordetella. Talk with your vet about these based on risk. The DHPP Is protecting against the main viruses that puppies are at risk to contract. Also, deworming is key at this age. Take a stool sample to the first vet visit.
