I need to know what forms must be submitted in order to file an appeal for an unlawful detainer decision?

Quick background- I owned & occupied this house. In Feb 2007 I refinanced was victim of fraud & never received closing paperwork until May 2007. Hired lawyer for that case but does not go to court until March 2009. In May 2007 I was laid off. The holder of the second foreclosed & it was sold. I was told by the lender of the first they could not foreclose. That was wrong & now its too late. The new owner got the house for 62,000, value was 500,000.

I had a share rental agreement with a tennant who lived in my house for over 10 years. I was given a notice to surrender but my tennant was never given a 30 day notice by new owner.
Reason for appeal- the exhibit introduced in court is not the same as the one on the summons.
2) they engaged in illegal activity. New owner conducted his own background check on us & alleges we are a gay couple & my tennant is not entitled to the same protection and laws that any other tennant enjoys. Not true! What forms must be filled out. Thank you!

Responding to the complaint. As a tenant, if you are served with a Summons — Unlawful Detainer (Form 982(a)(11)) and complaint concerning an eviction, you must respond to the summons. You may use Judicial Council Form 982.1(95), Answer — Unlawful Detainer, to respond and must file the answer form with the local court that issued the summons. You must also pay a filing fee when filing the answer form, unless you qualify for a fee waiver.
