I think what you are asking about is a Data Assessment Plan. These are often used by treatment centers, counselors, support people and case managers for people with substance/other addiction problems. Many times it appears as a check list that a support person can use to compare with other sessions or meetings to assess whether they believe the client is using again or not progressing with the program.

Under the "data" section the support person may look for things like subjective data like the client's thoughts or quotes, objective data like the client's mood or appearance and general notes on how the session went.

The assessment section would include things like what is the support person's understanding of the problem, what is their hypothesis about the situation, any results of screenings or testing that was given and the client's reaction to the treatment plan.

The last and most important part is the plan where the support person can ask questions about the current plan to see what is and isn't working and see what needs revision. They assign goals and objectives as well as setting up the next meeting time and future plan and goals.

Here - I just found a good little checklist of what should be included in a DAP Progress Note.
