You don't fill out a 1099, it is something that you get from an employer or someone for contract type work.

I know the employer does Form 1099! My question was what form do I need to fill out in order to receive a 1099 at year end.....consider me a subcontrator.

that would be a w-9 tax form

once your employer has that, they can send you a 1099 at the end of the year for your work.
