How do i find the form for 2003 hardship to get my refund

i just filed my 2003 taxes and the statue of limitations was up, I was told to file a form for hardship due to the fact that I have been in and out of the hospital and about to be homeless. I have the supporting documentation from the doctor and court but cant find the form do you know the number of the form and how i can get it
