Ineed to know the form# an non-taxpayer uses to file for Stimulus check

I am a nursing home ombudsman. The residents have been given different information or none by the social workers. I was looking for the form to take with me today. However I did not see it or neither do I know the number. I see with the information that SSI recipients will receive a check. What about those receiving Social Security?

It was a 1040a, but I think it might have need to been filed back in April. If you submitted it now, you may be delayed quite a bit in getting the stimulus rebate checks
